I'm having the same issue on Xbox version. I three-starred all Surf 'n' Turf levels in co-op, but I'm missing several of the star related achievements; Surf 'n' Turf', 'Skewer Rat', 'Smoothie Criminal'. 'Skewer Rat' and 'Smoothie Criminal' both show that they are 75% complete. Oddly, 'Carte Blanched' popped just fine.
